• Valid Passport (colour copy of front cover and photo page)
• Visa Documentation (if applicable)

• Qualifications and Certificates
• 2 proof of address
• A)Doctors; Original GMC Certificate and annual retention letter for GPs B)Nurses; NMC Pin Number and RCN
• Performers List Letter

• UK Serology reports, includes:
• HEP B
• Evidence of immunity to Measles, Rubella & Varicella
• BCG Scar sighting or evidence of immunity to TB
• IVS reports

• CRB
• BLS certificate or higher
• Medical Indemnity Insurance.
